UnitedHealthcare Announces Partnership With Waianae Coast Comprehensive Health Center

Honolulu (April 16, 2012) – UnitedHealthcare Community Plan Hawaii and Waianae Coast Comprehensive Health Center announced the formation of a partnership to support Waianae Coast Comprehensive Health Center’s "Patient Centered Healthcare Home" model for patients in Oahu.

“We are very pleased to have this opportunity to partner with Waianae Coast Comprehensive Health Center.  This partnership brings together innovative programs and community focused capabilities to align our organizations to achieve a shared goal of providing high quality health care services to our members,” said Bill Guptail, president of UnitedHealthcare of Hawaii.

The Patient Centered Healthcare Home model uses a proprietary web-based Population Registry that allows real-time health information sharing between hospitals and in-network clinics. The model also uses clinical rules to help health care professionals identify high-risk patients before a serious health event occurs, prevents readmissions to hospitals, and improves overall care for patients.

“Waianae Coast Comprehensive Health Center is committed to bringing the best healthcare possible to the communities we serve, this partnership with United is a significant step forward in the implementation of our innovative and community focused Patient Centered Healthcare Home model.” said Rich Bettini, CEO of Waianae Coast Comprehensive Health Center.

A community meeting will be held in the Waianae Coast Comprehensive Health Center main campus dining room Monday, April 16th at six p.m. to discuss the new healthcare home program. The public and press are invited to attend.

UnitedHealthcare serves approximately 21,000 aged, blind and disabled Medicaid members in the State of Hawaii’s QExA (QUEST Expanded Access) program and also serves 19,000 Medicare members in Hawaii.
