Introducing UnitedHealthcare® Connected®
We are pleased to introduce UnitedHealthcare Connected – which will be available as of March 1, 2014, to individuals who are eligible for both Medicare and Medicaid in: Columbiana, Cuyahoga, Geauga, Lake, Lorain, Mahoning, Medina, Portage, Stark, Summit, Trumbull and Wayne counties.
UnitedHealthcare Connected is a partner in the state of Ohio’s new integrated care delivery system for Medicare-Medicaid eligible enrollees (MMEs) called MyCare Ohio. UnitedHealthcare Connected is dedicated to improving the quality and efficiency of care for members through a person-centered, seamless approach, and lessening administrative burdens for providers through assistance with higher acuity patients and claims simplification.
As an industry leader in population health management, we will employ best practices for maintaining and improving members’ health. We’re also able to support a member’s decision to live in the least restrictive environment and maintain independence through our comprehensive and integrated care management programs.
We believe we can make care easier, more connected, personal and flexible for everyone involved. Our integrated program, UnitedHealthcare Connected for MyCare Ohio, combines Medicaid, traditional Medicare (Parts A and B), Medicare Part D (prescriptions), behavioral health, long-term care and home- and community-based services into a single plan.
A dedicated UnitedHealthcare Connected team will work collaboratively with family members, caregivers, health care providers, home- and community-based service providers and community organizations to help simplify care delivery and improve health outcomes for UnitedHealthcare Connected members.

- We are a pioneer and national leader in developing and operating integrated managed care plans for acute, long-term care and behavioral health.
- We introduced our first specialized long-term care program more than 20 years ago. Since then, we've expanded that program to include more than 200,000 members in nine states.
