Angel and Christy Delgado of Kearney, Neb., for years have struggled with illness, limited income and lack of housing and transportation. UnitedHealthcare housing navigator Sarah Hughes worked with the Delgados and community partners to find the couple safe, affordable housing and other social support services.
The statewide Housing Navigator program connects people enrolled in the Heritage Health Medicaid program who are at-risk of homelessness to housing and domestic resources. Since the program’s inception in August 2016, Housing Navigator has helped 162 Nebraskans address housing needs.
