A Value-based Care Success Story in North Carolina

In May 2017, UnitedHealthcare recognized over 100 North Carolina care provider groups with our 2016 PATH Excellence in Patient Service Award for their commitment to improving health outcomes for people enrolled in our Medicare Advantage plans. The PATH program impacts over 77,000 UnitedHealthcare Medicare Advantage members in North Carolina and is awarding $8M in quality-of-care bonus payments to qualifying provider groups that achieved performance metrics and successfully addressed care opportunities.

Additionally UnitedHealthcare renewed our Medicare Advantage Accountable Care Organization (ACO) relationship with Greensboro-based Triad Healthcare Network (THN). This relationship encourages greater alignment, clinical collaboration and outcomes-driven performance expectations with THN, all in an effort to improve health outcomes for the 11,000+ UnitedHealthcare Medicare Advantage members who receive care from THN doctors and care providers.
