The UnitedHealthcare Catalyst™ initiative brings the best of community partnerships and big data together to improve health outcomes in targeted communities and populations. This video explains why this initiative was launched.
The Catalyst model brings together the Council of Large Public Housing Authorities (CLPHA), public housing agencies, Federally Qualified Health Centers (FQHCs) and local community-based organizations to develop data-driven strategies specific to local communities. It employs a strategic process to reduce barriers and improve health equity and health outcomes through learning, assessment, planning, implementation, and refinement.
Here are two examples of communities that have implemented the Catalyst initiative to improve community health outcomes from the ground up:
- Watch this video case study to see how the initiative is removing barriers to care for the diverse immigrant and refugee populations of Northeast Ohio.
- This video case study illustrates how health and housing data can be leveraged to uncover patterns of disparity for BIPOC populations in King County, Washington.
