Bringing health equity into focus: UnitedHealthcare Catalyst™

The UnitedHealthcare Catalyst™ initiative brings the best of community partnerships and big data together to improve health outcomes in targeted communities and populations. This video explains why this initiative was launched.

The Catalyst model brings together the Council of Large Public Housing Authorities (CLPHA), public housing agencies, Federally Qualified Health Centers (FQHCs) and local community-based organizations to develop data-driven strategies specific to local communities. It employs a strategic process to reduce barriers and improve health equity and health outcomes through learning, assessment, planning, implementation, and refinement.
