We’re working to advance health equity by identifying and addressing system inequities, eliminating barriers to care, and supporting a modernized Medicaid program that brings together community-based organizations to address social determinants of health and deliver culturally relevant care.
Emphasizing person-centered care to improve population health
Our person-centered approach starts by building a trusting relationship with individuals and their families. We work to empower our members to be active participants in their own health and aim to remove barriers that stand in the way. Our highly coordinated services address physical and behavioral health needs, as well as social determinants of health including food, clothing, shelter and transportation.
Improving maternal health and child and family wellbeing
We are committed to innovation and collaboration that promotes accessible prenatal and postpartum care for pregnant individuals, as well as regular preventive care for all ages, from birth through adulthood. Our community and provider partnerships focus on reaching members in every corner of North Carolina and meeting their needs where they are.
