Rural Health Group, Inc. collaboration aims to improve children’s health outcomes in North Carolina
UnitedHealthcare Community Plan of North Carolina strives to consistently improve the physical and behavioral resources available to children across the state. Approximately 4 out of 10 children in North Carolina live in low-income homes.1 Their families often contend with challenges including access to health care, affordable housing, transportation, healthy food and safe public parks for play and exercise, all factors that increase the risk of poor health outcomes. Recognizing these barriers, UnitedHealthcare Community Plan of North Carolina has invested $311K in Rural Health Group, Inc. (RHG) to expand child-centered health care interventions that improve academic achievement while also addressing the physical and behavioral health of children and youth.

RHG, a nonprofit Federally Qualified Health Center, serves the underserved populations of northeastern North Carolina, offering primary medical, dental, OB/GYN and behavioral health care.2 Founded as a community-driven initiative in Northampton County, RHG has expanded its services to five counties, providing nearly 40K individuals with affordable health care. RHG ensures that no one is turned away due to their inability to pay by providing a sliding fee scale based on income and family size.
UnitedHealthcare Community Plan of North Carolina’s investment aligns with RHG’s commitment to providing students, teachers and administrators with child-centered health care interventions that improve academic achievement while addressing the biophysical-psychosocial issues facing each child.
The funding will be allocated to:
- Registered nurse (RN): The RN will work directly with children and families to address physical health needs, focusing on preventive care such as wellness exams and immunizations in addition to managing acute illnesses. Care will be coordinated with local primary care providers using telehealth technology.
- SBHC director: The director will collaborate with school districts across the five counties RHG serves, developing systems and resources to meet the needs of children and families.
- Case manager: This role will lead mental health outreach, connect families to resources, and development initiatives.
- Licensed clinical social worker (LCSW): The LCSW will serve as the primary provider of expanded behavioral health services.
- Telehealth equipment: Additional telehealth units, hot spots and tablets will extend medical and behavioral health services to more campuses, increasing access for students.
- Marketing: Funds will support promotional efforts to raise awareness of RHG’s services, including electronic ads, brochures, promotional materials and outreach events.
The investment is designed to achieve several measurable outcomes within a 12-month period:
- Mental health support: Provide mental health support for 4-8 students, 4 days a week, and group sessions for at least 30 students per site monthly.
- Mental health ratings: Help at least 50% of students improve their mental health, as measured by survey data.
- Enhanced family communication: Support at least 50% of families in improving communication between parents and children.
- Replicability: Document the processes and lessons learned to replicate the program at other sites with recommended improvements.
Students began accessing these expanded services in December 2024.
