UnitedHealthcare Community Plan of Texas has been working together with religious groups, community-based organizations and health centers such as Su Clinica to help members navigate the Medicaid redetermination process. Outreach teams are visiting health fairs, food banks, church events and other gatherings to bring health education directly to the community.
