Lisa Granieri, a UHC RN care coordinator, first met with Community Plan member Robert Wilkie to provide him with diabetes management support and connect him to dental resources.
“Flash forward a couple years and it was the day before Hurricane Helene was coming, and I remembered the one time I visited him in his home and did all the diabetes instruction, that he lived right next door to the store that I happened to see on the news the day before the flood,” said Granieri. “And there was a car floating in the parking lot of this grocery store, and I gasped and said, that’s Mr. Wilkie’s house.”
Recognizing Wilkie’s proximity to the hazardous weather conditions, Granieri reached out to UHC care team colleagues to proactively contact Wilkie and check on his well-being. At the time of that phone call, Wilkie said rising floodwater was nearing the top step of his home.
“It went from about knee deep to about neck deep in a matter of seconds,” he said.
